Multimodalitné zobrazenie vrodených anomálií koronárnych artérií a neaterogénnych koronárnych chorôb

Anomálie koronárnych artérií sa všeobecne v populácii vyskytujú veľmi zriedkavo. Môžu byť ako izolovaná

anomália alebo ako súčasť vrodených chýb srdca. Len 20 % je potencionálne klinicky signifikantných, môžu

sa prezentovať život ohrozujúcimi príznakmi ako napr. myokardiálnou ischémiou, malignou komorovou

arytmiou alebo náhlou kardiálnou smrťou. Obyčajne sú zachytené ako náhodný nález počas CT, resp. katetrizačnej

koronarografie. Predstavujú druhú najčastejšiu príčinu smrti u mladých športovcov po hypertrofickej

obštrukčnej kardiomyopatii. V tomto prehľadovom článku sa pre zjednodušenie a klinický význam

zameriavame na najčastejšie sa vyskytujúce anomálie koronárnych artérií, ktoré môžu byť klinicky významné

a vyžadujú si intervenciu, či už katetrizačnú, alebo kardiochirurgickú. Zároveň popisujeme silné a slabé

stránky jednotlivých modalít pri zobrazení koronárnych artérií. Samostatnú pozornosť venujeme zápalovým

ochoreniam steny koronárnych artérií neaterogénneho pôvodu, ktorých hlavným predstaviteľom v detskej

populácii je Kawasakiho syndróm.
